the criteria diagnose left bundle branch block on electrocardiogram:
the heart rhythm must supraventricular in origin
the qrs duration must ≥ 120 ms
there should qs or rs complex in lead v1
there should notched ( m -shaped) r wave in lead v6.
the t wave should deflected opposite terminal deflection of qrs complex. known appropriate t wave discordance bundle branch block. concordant t wave may suggest ischemia or myocardial infarction.
there partial blocks of left bundle branch: left anterior fascicular block (lafb) , left posterior fascicular block (lpfb). refers bifurcation of left bundle branch.
diagnostic consequences
the presence of lbbb results in electrocardiography (ecg) cannot used diagnose left ventricular hypertrophy or q wave infarction, because lbbb in results in widened qrs complex, , changes in st-t segment consistent ischemia or injury.
